Allergies & Anaphylaxis

If your student has an allergy that requires medication to be taken at school, please fill out and have your physician sign the order for the medication(s) to be in the school and how the medication should be administered.

Medication

Medication that is to be kept in the health office needs to be:

Prescriptions:

Over the counter:

Example: EpiPen

Must be in a prescription bottle labeled with: student’s name, name of medication, dose/strength, instructions for administration, and expiration date.

Example: Benadryl

Must be new, sealed, labeled upon delivering to the school.

Student’s who will “self carry” their medication must have an approved medical plan AND are allowed to carry only enough medication for one day at a time.
