Instructions

The Free & Reduced Meal application can be filled out online through your Parent Infinite Campus account or a physical copy can be picked up at any school location or the District Office. Additionally, you can downloaded the application below.

Online Application

  1. Login to Infinite Campus. If you do not have a login or have forgotten your password, please contact a school secretary.
  2. Within Infinite Campus on the left menu, select More.
  3. On the resulting page, select Meal Benefits. There will then be a blue link to start the application process.

If you have any questions about how to define a household member, what counts as income, etc. Please see the “How-to” article below.

Physical Application

  1. Pick up the application at any school location (including the District Office) or download and print off via the buttons below.
  2. Use a pen to complete the form.
  3. Return the form to LCSD#2.

If racial/ethnic background is not reported, a visual identification of the child's race and ethnicity will be made.

In accordance with Federal law and U.S. Department of Agriculture (USDA) civil rights

To file a program discrimination complaint, a complainant should complete a Form AD-
3027, USDA Program Discrimination Complaint Form, which can be obtained online, at
www.usda.gov/sites/default/files/documents/usda-program- discrimination-complaint-
form.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter
addressed to USDA. The letter must contain the complainant’s name, address,
telephone number, and a written description of the alleged discriminatory action in
sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the
nature and date of an alleged civil rights violation. The completed AD-3027 form or letter
must be submitted to USDA by:
